When the provider names the disorder, use it: F41.1 generalised anxiety disorder, F41.0 panic disorder (episodic paroxysmal anxiety, without agoraphobia), F40.01 panic disorder with agoraphobia, F40.10 social anxiety disorder, F40.2- specific phobias, F41.3 other mixed anxiety disorders, F41.8 other specified anxiety disorders. Anxiety due to a documented medical condition is F06.4 with the condition coded first; substance-induced anxiety is coded to the substance (for example F10.180 alcohol abuse with alcohol-induced anxiety disorder).
Anxiety with depression: one code or two
When the provider documents anxiety and depression as two diagnoses, report two codes: F41.9 (or the specific anxiety code) and F32.A, F32.9 or the specific depressive code. When the provider specifically diagnoses 'mixed anxiety and depressive disorder', that is the single code F41.8. Adjustment disorders are neither: F43.22 adjustment disorder with anxiety, F43.23 with mixed anxiety and depressed mood. The distinction is the provider's wording, and a coder should not collapse two diagnoses into F41.8 or split F41.8 into two.
PTSD, acute stress and the pediatric codes
Post-traumatic stress disorder is F43.1- (F43.10 unspecified, F43.11 acute, F43.12 chronic), acute stress reaction F43.0, and neither is an F41 code even though anxiety is a feature. Separation anxiety disorder of childhood is F93.0. Anxiety documented as a symptom during a medical encounter (pre-procedure nerves, for example) does not warrant a diagnosis code unless the provider diagnoses a disorder; R45.- covers symptoms involving emotional state where a symptom code is genuinely what is documented.
